Paste this in exactly as the complaint page formats it. Go to the complaint form, type your address into its Address box, pick your address from the dropdown it shows, then copy the finished text out of that box and paste it here. It must match character-for-character or the city may not record your address.

Code copied — now set up your bookmark
First, get your address string: open the complaint form, type your address into its Address box, tap your address in the dropdown it offers, then copy the finished text out of that box and paste it into the Address field above. Then tap Copy Fill Code again.
1. Open the city complaint form in Safari.
2. Tap the
Share icon
→ Add Bookmark. Name it "Quiet Skies Fill."
3. Go to
Bookmarks,
tap Edit next to your new bookmark.
4. Replace the whole web address with the code just copied — tap the field, tap Paste.
5. Save. From now on: on the complaint form or the "submitted" page after, tap the saved bookmark itself in your Bookmarks list to return to auto fill out the form or return to the form from another webpage.
6. After the complaint is submitted, tap the bookmark again to bring back up the form. Tap the bookmark again to auto-fill it and it's ready to submit another complaint.
Tip: if your Bookmarks icon keeps disappearing behind a "•••" button, go to Settings → Apps → Safari → Tabs, and switch the layout to "Bottom" or "Top" (not "Compact"). This keeps Bookmarks always on screen and one tap away.

On the "complaint submitted" page, it takes you straight to a fresh blank form — tap the bookmark once more there and that fresh form fills in too. Date and time are captured fresh every time you tap it.
6. After the complaint is submitted, tap the bookmark again to bring back up the form. Tap the bookmark again to auto-fill it and it's ready to submit another complaint.

⚠️ Important: always tap the bookmark from your Bookmarks list itself — never type or tap it from the address bar, even if it shows up as a suggestion there. Safari blocks it from that spot and it will silently do nothing.
